Sentinel Event Review

We have a skilled staff of both nurse and physician consultants that will review a sentinel event, on site and assure that the investigation and documentation are complete and accurate. Additionally, the consultant will meet with the team to conduct an independent and timely review of the sentinel event.

Root Cause Analysis (RCA) and Failure Mode Effect Analysis (FMEA)

We will review the RCA and FMEA to assure that it was correctly completed and will pass JCAHO review.

Root Cause Analysis and Failure Mode Effect Analysis Training

Experience has shown that hospitals are still not conducting effective and in-depth Root Cause Analysis or Failure Mode Effect Analysis that 1) actually determine the root cause and 2) will pass the surveyor review. Staff resources often prevent these analysis from being completed on a timely basis which creates serious liability issues.
